I/WE the undersigned assume all risks associated with the tournament and release LEE KING BENEFIT TOURNAMENT, the sponsors and tournament officials from all claims of injury and/or damage incurred in connection with this tournament. I/WE also understand that the tournament committee reserves the right to reject my/our entry for any reason, and upon such rejections, refund my/our entry fee.
We further agree to take a polygraph test should I/WE be asked.
